Want the vaccine but can’t make a scheduled drop in time? No problem!

Call or email Kelly Minor at 615-487-2036, kminor@alivehospice.org or Danielle Hopkins at 615-346-8550, dhopkins@alivehospice.org to schedule a time to get the vaccine.

Get your flu shot for a chance to win!

We will be awarding a total of ten $100 cash prizes. Individuals who receive a flu vaccine at one of our flu clinics OR provide proof of flu vaccine for the 2021/2022 season are eligible to submit their name for a drawing for $100.

To be eligible, you must get your flu shot at one of the Alive flu shot clinics OR provide proof of flu vaccination by Friday, October 29, 2021. Drawing will be held on Monday, November 1, 2021.
